Documentation in a patient's record is difficult for many nurses because information has to be both condense and clear. I think that proper documentation is a large grey area for most nursing staff.

I try not to write too much in nurses notes. In our facility, we still use flowsheets and write daily notes on each patient. Next time i will definately remember to follow the rules listed above and try to spread the news.

Though my documentation is not perfect, I often find myself in disagreement with others when it comes to points such as, not mentioning staffing or delegation matters in a patient's chart. Also I'm going to suggest an refresher inservice on proper documentation to my Nurse Manager.

As such, these written findings is a mechanism of communication to other healthcare members. In addition, the value of quality nursing care can be obtained from the manner in which it was conveyed in a patient's medical record. I appreciate this blog and will share it with my students. Thank you Bridgette for sharing.

Even though handwriting was difficult to read at times prior to the electronic chart, the computer "spell checker" is not accurate either! Thanks Bridgette, I really enjoyed reading this!
