hi Libby - searching this forum i found your email regarding OT eval forms.  We have a school based pediatric agency servicing eastern Long Island (NY).  We have an eval form but always looking for new and improved ideas to merge with our form (esp when incorporating the new framework).  Our forms were developed with uniform terminology and clincial performance components (yes we should update!).  I'd love to see what you're using and would share our forms with you.
