Hellom Does anyone know if there is a requirement to fill out a Code Blue sheet if the patient coded in the OR, right after induction. The anesthesiologist has a very detailed record of what was done, but i wonder if a code blue sheet would still be required.  Most of the OR nurses i spoke with say that there is no Code Blue sheet in the OR - that the anesthesia record is all.  However, I wonder if there isn't a protocol / requirement for code blue sheet to be filled out.  
Any input or direction would be greatly appreciated.
