Could anyone throw some light on the issue of EMR? With its advent, will medical transcription cease to exist or will it just be turned into a menial data entry job? How will EMR affect medical coding and medical billing services? What is the future of medical transcription professionals with the advent of EMR?

Locum Tenens

Our physicians allow monthly rotations of a resident physician program in our facility. These residents are always with the physicians as they are seeing the patients in the clinic. We only have a issue when the residents see the patient in the ER and the supervising doctor is not always available and the resident is seeing the patient sometimes alone without supervision. This would be nights and weekend call. My question is on the Q6 modifier and the locum tenens requirements. Have other offices used this modifier and has it applied to residents? Any help would be appreciated. My thoughts are that this senario would not apply for locum tenens. Thank You!

Why is it that the supplement programs that patients choose to go with their Medicare, raise the plans without letting thee patients knows before hand?

Also, why do you think so many of the insurances companies require prior authorizations for medications that they have taken for a long time?

Hi, I'm in the process of changing careers, and would like to switch to health care. I have a bachelor's degree in graphic design, and am contemplating either medical coding or medical transcription, and would like to channel my efforts into the program which is more marketable and has more job opportunities upon completion. Office visit when the patient is not present

We have a delima in our office. Our physician will counsel a family member about a patient but the patient is not present. Is there any guidelines out there directing you on how to bill for these services. My thoughts are you would code an establish patient visit, document your time and the fact that the patient is not present, then document everything that was discussed. Please if anyone has any insight on this let me know.

Incision & Drainage Procedure

Is it appropriate to bill the I&D CPT 10060 when the provider did not actually make an "incision" as the wound was already open? See documentation below:

Knee examination does reveal a moderate amount of swelling in the prepatellar area and does not appear to have any intraarticular effusion. He does have a small open lesion in the front of the knee just below the tibial plateau just to the right of the midline. There is a significant amount of serosanguineous drainage expressed from this. Using forceps a mucus plug is removed and then there is a mild amount of purulent/serous fluid which is obtained from it then. Culture swab is obtained. Thank you inadvance for any clarification you can provide.

Another example would be an abscess that is unroofed with a 16 gauge needle and then expressed of matter.

I was wondering if anyone knows a reference of clinical examples that support or dispute the usage of modifier 25. I would just like something to reference something that could help me to determine in complex situations when the usage of modifier 25 would be appropriate or not. I'm sure something has to be out there, but would prefer it to come from a concrete source (i.e. AMA [highly preferred]). Thank you!
