Code 58563 is the correct code for the hysteroscopic ablation. Code 58558 is the correct code for hysteroscopic D&C. The code pair 58563 and 58558 cannot be reported together. CPT code 58558 is a component code of 58563 and therefore the only CPT code reported for your scenario is 58563.

A patient comes in to have a Hysteroscopy, D & C, and Thermal ablation of the uterus in which you would use (58563)
My question is in CPT code 58558, the semicolon is after the Hysteroscopy surgical; Would I be able to code the D&C (58120) with modifier 59 extra, because it does not seem to be fully captured in code 58563 since the common part of the code is BEFORE the semicolon and does not seem to include the polypectomy or D&C in the codes that follow??
