Esentially the RN is acting as a scribe. Then the RN does the dictation, and both of us sign the dictation.&amp;nbsp; &lt;/P&gt;

We're preparing an article about barriers to effective obesity treatment, and we'd like to hear from you with an answer to the following question: What is your best piece of advice for weight loss?</description></item><item><title>BNP and CHF</title><link>http://community.advanceweb.com/forums/thread/79266.aspx</link><pubDate>Mon, 11 Mar 2013 21:09:39 GMT</pubDate><guid isPermaLink="false">06d5312c-37b9-406e-be84-460d8d21f4fc:79266</guid><dc:creator>Kathryn Hanson</dc:creator><slash:comments>1</slash:comments><comments>http://community.advanceweb.com/forums/thread/79266.aspx</comments><wfw:commentRss>http://community.advanceweb.com/forums/commentrss.aspx?SectionID=177&amp;PostID=79266</wfw:commentRss><description>&lt;p&gt;I am writing to get feedback on a commonly ordered test--BNP in evaluation of CHF status. &lt;/p&gt;&lt;p&gt;Recently had&amp;nbsp;a patient with diastolic CHF and multiple comorbidities including stage IV renal failure, aspiration issues, restrictive COPD with&amp;nbsp;emphysema, etc..&amp;nbsp; Very complicated and delicate scenario.&lt;/p&gt;&lt;p&gt;In absence of edema and weight gain,&amp;nbsp;but presence of respiratory symptoms (basilar rales, wheezing), is there benefit of &lt;br&gt;"rechecking" BNP in a patient with chronically elevated BNP? &lt;/p&gt;&lt;p&gt;In this case, I essentially checked&amp;nbsp;a BNP&amp;nbsp;&amp;nbsp;to "see if it had risen significantly" when trying to decide whether adjusting meds to decrease preload and/or afterload would be of benefit (along with CXR, CBC, etc).&amp;nbsp; In the end decided upon changing up nebs, extending abx, adding short burst of steroid, etc, but didn't fiddle with diuretics or BP meds (sats good, VS stable, etc).&lt;/p&gt;&lt;p&gt;In presence of severe renal disease, I am hesitant to jump on the increasing diuresis bandwagon if there aren't other clinical indicators to do so. I was looking to the BNP to see if further imaging (ie echo) or referral back to cardiology seemed warranted (as well as possible med adjustment). The whole not having a furnishing license has really stopped anyone from hiring me. I've been on over 50 interviews with no offers. I've even had some doctors tell me I should have been a PA because then I could get a job right after certification. I also can't get a RN job because nurse educators/recruiters say I'm overqualified and therefore undesirable. &lt;P&gt;&amp;nbsp;Where I work, we have to rearrange our schedule and work more days in a long stretch to have a long stretch of days off.&amp;nbsp; If we are out sick, we have to make up the day.&lt;/P&gt;

(I'm in primary care and addiction medicine in a community health clinic). This patient had been previously diagnosed by rheumatology as having possible RA vs scleroderma but had not followed up with them in over 2 years and had been on plaquenil all that time. She came in complaining of severe bilateral joint pain in her shoulders which were extremely tender to touch with limited ROM. I tried to call rheumatology but was unable to reach anyone for consult, so decided to start the patient on methotrexate with leucovorin and get her in to see the rheumatologist ASAP. However, though I sent the referral, it took them 3 months to actually see her. I was monitoring her CBC and CMP as required while on methotrexate therapy and she seemed to be responding well and her labs were all normal. I had realized somewhat late that typically methotrexate is prescribed by a rheumatologist and not a PCP, but she seemed to be doing well so I figured I might as well continue. &lt;/p&gt;&lt;p&gt;&amp;nbsp;Well, the rheumatologist recently emailed me after the patient missed her appointment and we discussed the current treatment. She told me that while she could tell I was trying to help the patient, this was NOT appropriate, the dose was too low and the leucovorin would counteract the effects of the methotrexate anyway (Epocrates led me astray: said it should be prescribed with the methotrexate but I guess it's better to do folic acid 1mg). &lt;/p&gt;&lt;p&gt;Although the patient wasn't hurt, I feel really bad about it, and I just need to hear some mistakes other people have made in their careers and what the process was like of getting through the beginner phase.&lt;/p&gt;&lt;p&gt;Thanks &lt;br&gt;&lt;/p&gt;</description></item></channel></rss>
