$50 for participation in a Type 2 Diabetes Market Research Study--PLEASE HELP SPREAD THE WORD

We are reaching out specifically to Health Care Professionals who treat Type 2 Diabetes in their patients. We are particularly interested in reaching out to Nurses, Nurse Practitioners, and Registered Dieticians that are part of the Diabetes Educator Community. Your participation is vital and critical in helping us to develop effective Programs and Tools for patients with Type 2 Diabetes.

This study offers a $50 honorarium upon completion. We appreciate your time and participation in this Market Research Study. To participate in the survey simply email DCSurvey@knology.net and I will provide a link for each individual to participate. These are individual links. At the end of the survey, you will be asked for Name and Address. This information will be used for no other purpose than to send the honorarium for participation and will not be collected nor used in any other way..
