can anyone tell me the process they are using within the snf environment to change a resident from med A to med B?  are you simply writing a clarification order noting the change? or are you d/c'ing the patient on the med a paperwork with discharge orders written, and then re-evaluating on part b paperwork with new eval and clarification orders?  thanks in advance for responses.
